Consent form questions

You will be asked to complete a version of this form on the day of your tattoo at the studio. Reading these questions now can highlight any potential barriers there may be to you getting tattooed. If you would answer yes to any of these questions, please ask me about it on your enquiry form.

You will be asked these on your consent form, and need to agree to all of these points, apart from the last two. I may still be able to tattoo you if you have moles/freckles/scarring in the area you want to be tattooed. And you do not need to consent to having a picture of your tattoo taken to be tattooed by me, and I will ask before taking a picture of your tattoo/s

I consent to this data being held on file for a period of at least two years, as required by licensing regulations.

I understand that tattooing means the insertion into the skin of ink designed to leave a permanent mark.

To my knowledge, I do not have a physical, mental or medical impairment or disability which might affect my wellbeing as a result of my decision to have a tattoo.

I give my full consent to the application of the tattoo and to any actions or conduct of the artist reasonably necessary to perform the tattoo procedure.

I understand that every care will be taken to ensure that the procedure is carried out in a hygienic way, which includes the use of single use and pre-sterilised equipment.

I acknowledge that infection is always possible as a result of being tattooed, particularly in the event that I do not follow after-care instructions, and that any such infection is not the responsibility of the artist.

I will follow the verbal and written after-care instructions which have been given to me.

I acknowledge it is not possible for the artist to determine whether I might have an allergic reaction to the pigments or processes used for my tattoo, and I agree to accept the risk that such a reaction is possible.

I understand that because tattoos are created by hand and heal differently on each person, the appearance of the final tattoo may differ slightly from the original artwork and change over time.

I agree to release, discharge and hold harmless the artist from any and all claims, damages and legal actions arising from or connected in any way with my tattoo and the procedures used to apply my tattoo.

I am not pregnant or breastfeeding.

I am not under the influence of alcohol or drugs, and I have requested this tattoo of my own free will.

I understand that I may be asked to leave the studio if I demonstrate any language or behaviour that is racist, homophobic, misogynistic, ableist, fatphobic, transphobic or otherwise discriminatory, hateful, shaming or harmful.

During my appointment at FORT, I agree to be mindful of conversation topics that have a high chance of being triggering. (Triggers are very different for different people so it’s impossible to always avoid this, but consider checking in with the person you’re speaking to before discussing topics that contain details of trauma. Also be mindful of others in the studio who might be within earshot.)

I do not have medical or skin conditions such as, but not limited to: acne, scarring (Keloid) eczema, psoriasis, freckles, moles or sunburn in the area to be tattooed that may interfere with said tattoo.

I agree to allow any photographs taken by the artist to be used for promotion such as, but not limited to: social media platforms, websites, other marketing materials, etc.

Disclaimer

No tattoo can be 100% perfect, but I will always try my best to meet your expectations. All skin is different, and this can be reflected in the healing process, especially on the hands, fingers, wrists, feet and ankles. One touch up in these areas will be free, subsequent touch ups will be at my minimum price.
